WAIVER OF LIABILITY AND ASSUMPTION OF RISK
I, the undersigned, acknowledge and accept that my participation in physical activities, use of equipment, facilities, common areas, or any other services offered by RESULTS BOSTON GYM involves inherent risks, including but not limited to physical injuries, illnesses, property damage, and even death.
By signing this document:
I fully assume all responsibility for any accident, injury, physical harm, material loss, or other incident that may occur to me within the gym premises, in its surroundings, or while using any of its equipment or services.
I hereby fully and permanently release RESULTS BOSTON GYM, as well as its owners, employees, representatives, trainers, and affiliates, from any and all legal, civil, or financial liability arising from injuries, losses, or damages sustained during the use of its services or facilities.
I declare that I am in adequate physical condition to participate in the gym’s activities. I understand that it is my responsibility to consult a physician before beginning any training program.
I understand and accept that the use of the gym’s facilities and equipment is at my own risk, and that RESULTS BOSTON GYM does not guarantee my safety nor is responsible for supervising me during my time at the gym.
I agree to follow all rules, regulations, and recommendations of the gym and its staff. I understand that failure to do so may result in my removal from the premises without the right to any refund.
I authorize the use of any images or videos in which I may appear, taken within the gym, for promotional or advertising purposes, without any compensation.

RENUNCIA DE RESPONSABILIDAD Y ACEPTACIÓN DE RIESGOS
Yo, el abajo firmante, reconozco y acepto que mi participación en actividades físicas, uso de equipos, instalaciones, áreas comunes o cualquier otro servicio ofrecido por RESULTS BOSTON GYM, implica riesgos inherentes, incluidos, entre otros, lesiones físicas, enfermedades, daños a la propiedad e incluso la muerte.
Por medio de la presente:

Asumo plena responsabilidad por cualquier accidente, lesión, daño físico, pérdida material u otro incidente que pueda ocurrirme dentro de las instalaciones del gimnasio, en sus alrededores o al usar cualquiera de sus equipos o servicios.

Libero completamente y para siempre a RESULTS BOSTON GYM, así como a sus propietarios, empleados, representantes, entrenadores y afiliados, de cualquier tipo de responsabilidad legal, civil o económica derivada de lesiones, pérdidas o daños sufridos durante el uso de sus servicios o instalaciones.

Declaro estar en condiciones físicas adecuadas para participar en las actividades del gimnasio. Entiendo que es mi responsabilidad consultar a un médico antes de iniciar cualquier programa de entrenamiento.

Entiendo y acepto que el uso de las instalaciones y equipos es bajo mi propio riesgo, y que RESULTS BOSTON GYM no garantiza mi seguridad ni se hace responsable de supervisarme durante mi estadía.

Acepto seguir todas las normas, reglas y recomendaciones del gimnasio y su personal. En caso de no hacerlo, comprendo que puedo ser expulsado de las instalaciones sin derecho a reembolso.

Autorizo el uso de imágenes o videos en los que pueda aparecer, tomadas dentro del gimnasio, para fines promocionales o publicitarios, sin compensación alguna.
